The tolerance and safety was very well tolerated, with no untoward symptoms reported. There was only one subject with an adverse effect of erythema of the entire face on Day 1 of the study that subsided in 2 days without stopping the use of either lotion (GSSG cream and placebo lotion). The limitations on the study was the small sample size of 30 health Filipino women and the short duration of the study, which was only 10 weeks and there was no follow-up results obtained after the study was completed (i.e., the follow-up results were not mentioned beyond the study duration of 10 weeks). In addition, the study may not extrapolate for other ethnic groups. Nevertheless, the results for skin lightening showed progressive and significant reduction in melanin index values over 10 weeks at the GSSG cream application sites as compared to the placebo sites. The other results demonstrated suggested that significantly higher skin moisture for GSSG sites than placebo sites in Weeks 8 and 9 of the study; significantly lower curvature index values at GSSG sites than placebo sites in Weeks 6 and 10 of the study; significantly lower Keratin index values at GSSG sites than at placebo sites from Weeks 6 to 10 of the study; and no significant difference between the two sites regarding the elasticity index values.

Glutathione orally, however, used as an antioxidant has a standard dosage for oral intake (dependent on the person’s body weight) is usually doubled in dosage if it is used as a skin-whitening agent. The parameter of standardized digital photographs was quantitatively evaluated for Ultraviolet (UV) spots, pores, and evenness on the left and right sides of the face. The study groups tolerated the treatment very well and the only adverse effects were flatulence from one subject in the Glutathione group during the initial days and constipation reported by one of the other subjects receiving the placebo group. However, again the follow-up results were not obtained beyond the 4 weeks of study duration and length (i.e., follow-up for evaluation of persistence of efficacy is lacking). Some limitations in the study included its short study period of 4 weeks and a small sample size of 60 healthy medical students.

There was another study by the Authors Handog et al published in 2015 that displayed an open-labeled, single-arm pilot study for oral lozenges for buccal mucosal absorption of Glutathione, which studied a group of 30 healthy women (aged 22 to 24 years of age) with Fitzpatrick skin types IV or V, with melanin indices of more than 20 (maximum value = 99). The methodology described the cohort group using one lozenge (500 mg) per day that was kept in the mouth against the inner cheek (buccal mucosa) and clinical evaluation was performed at baseline and every two weeks over a periods of eight weeks by a portable Mexameter. The parameters evaluated include the Melanin index (by Mexameter) and measurements were done in triplicate and the mean was taken at two sites: Sun-exposed areas such as the extensor surface of the right wrist and sun-protected area in the mid-sternum. The results showed that the melanin index in all subjects (100%) receiving Glutathione at both sun-exposed and sun-protected sites had significant reduction in melanin index (p < 0.0001). The Global Assessment demonstrated twenty-seven of the subjects (90%) noted moderate skin lightening and three noted mild skin lightening (10%). The treatment group in those who completed the study tolerated the Glutathione oral lozenges very well. There were no adverse effects amongst those who completed the study but one drop out complained of soreness of gums due to the lozenges. Some limitations on the study included no follow-up investigations beyond the 8-week duration of the study. Follow-up for evaluation of persistence of efficacy is lacking. It was also a very short study period of 8-weeks with only 30 subjects on a small sample size that comprised of healthy young adults.

Nevertheless, there are relatedly sometimes very unusual and certainly infrequent complications of peripheral intravenous (IV) therapy insertions even with standard of care techniques utilized by a competent and seasoned IV operator or medic that may include not only anaphylactic reactions, reactive airway diseases, teratogenicity, and anxiety but also thrombosis may occasionally happen when the platelets cling to the tunica intima of the vein due to vessel injury during venipuncture with the designated vein appearing painful, red and swollen in addition to a sluggish IV infusate flow rate; extravasation inappropriately happens rarely when the venipuncture device is dislodged from the vein with signs and symptoms of swelling at and above the limb, discomfort, burning or pain at the site, tightness at the site, cooler temperature at the site, blanching at the site, and absent backflow of blood; catheter dislodgement unluckily arises when the catheter backs out of the vein and the IV therapy solution may infiltrate, the catheter will be backed out further than normal from the vein and the tape may be loose; nerve, tendon or ligament damage inopportunely follows due to improper venipuncture technique, tight taping or improper arm board application with the patient experiencing tremendous pain at the insertion site, numbness, muscle contraction and finally may display paralysis, numbness and deformity; circulatory overload can unfortunately strike when the IV therapy roller clamp is loosened and the infusate is allowed to run into the vein quickly, especially in patients with moderate to severe heart failure, renal disease, or liver disease which may cause the patient to be more anxious, experience respiratory distress and shortness of breath (e.g. dyspnea), crackles in the lung bases upon auscultation (e.g., interstitial lung disease with flash pulmonary edema), increased blood pressure (e.g., increased intravascular volume) and neck engorgement (e.g., increased jugular venous distention due to fluid overload); air embolism may seldom and rarely occur when the IV therapy solution container runs empty and the added container pushes enough significant air down the line into the patient causing the patient to experience respiratory distress and shortness of breath (e.g. dyspnea), unequal breath sounds upon auscultation, a weak and thready pulse, increased central venous pressure, decreased blood pressure (e.g., hypotension due to an obstructive shock case), and loss of consciousness or altered mentation; and local cellulitis, systemic infection or bacteremia can infrequently transpire as a result of phlebitis, poor taping that allows the venipuncture device to move in and out of the vein, prolonged dwell time of the catheter, and failure to maintain aseptic technique during IV insertion or site care and many hours later the patient may experience malaise, fever, and chills which may turn into critical sepsis.
